When we have spoken and you have an appointment date, please download the three forms: Intake, Disclosure,  and Policies onto your computer.

The intake form requires a few minutes to fill out.

Carefully read the Disclosure and Policies  documents,  then sign (electronic signature is fine) and date both.  

Then return all three BEFORE OUR APPOINTMENT to drkaren@drkarenphd.com .

Don’t forget to arrange to set up a  VENMO account so I can bill you directly after the appointment.
